| A transfer of ownership shall be submitted to DPW within 30 days of UST ownership change or change in operator. The Transfer of HMUSP form, Transfer fee, Unified Program Facility forms BUSINESS (DOC, 102 KB), FACILITY INFORMATION (DOC, 174 KB), TANK INFORMATION (DOC, 162 KB) and MONITORING PLAN (DOC, 226 KB), Certification of Financial Responsibility, Response Plan and Designated Operator form shall be submitted and all outstanding fees shall be paid prior to Transfer being approved.
