Address requests are handled in EPIC LA: https://epicla.lacounty.gov/energov_prod/SelfService/#/home
Once you have registered, you can click on the "Apply" tile on the home screen. In the "Application Assistant" type "address" in the search box, and "Address Request" will appear, which will take you through the application process.
