Each Permittee is required to submit its Individual Annual Report, required attachments and signed Certification page. Permittees must also complete the Best Management Practices (BMPs) Implementation form and NPDES Program Summary form for each applicable watershed. Permittees will only be able to submit the items listed above between July 29 and September 2, 2008. |

- Download the Individual Annual Report form (Attachment U4)
and the Certification form.
- Complete Individual Annual Report (Attachment U4).
- Obtain the appropriate signature for the Certification page and convert it to an electronic file (PDF).
July 29, 2008 - September 2, 2008
- Register with this site to obtain a login code.
- Complete on-line the Best Management Practices (BMPs) Implementation form for each applicable watershed.
- Complete on-line the NPDES Program Summary form for each applicable watershed.
